Foot jobs, at least for me, encompass 3 things.

I agree to some extent, footjob's will clearly be appreciated more by a foot fetishist, it's part mental seeing the object of your desire massaging your shaft until you explode all over it is very hot. But only to a foot lover.
Some people may find FJ's uncomfortable or even disgusting but you can't argue that a footjob does feel very different to a handjob, I think the soles are softer which provides a different sensation.
